A separate application is required for each person.
If you are hand-writing your application, please print clearly.
Since there is a waiting list to attend, it is important to submit your application as soon as possible.
The cost (including meals and lodging) is $150, collected at the end of your weekend.
The completed applications will be kept on file untill you attend but for no more than 2 years. If you have not attended within 2 years you will need to fill out a new application.
You will be notified of the first available opening. Please do not let cost keep you from attending. Assistance is available. God Bless You!
